Through case coordination, we aim to:
- Improve access to timely, appropriate services and supports across sectors
- Reduce barriers to engagement and strengthen ongoing connection to care and community resources
- Support continuity and coordination, including warm transitions between providers to minimize gaps and duplication
- Promote recovery, resilience, wellbeing, and client autonomy through person-centred, trauma-informed practice
