As a community navigation organization, Community Connection helps individuals, families, and communities identify, understand, and effectively access the programs and services within the human services system.

We use enhanced tools to improve how we collect, share, and act on information for vulnerable clients—integrating with Ocean eReferral, piloting comprehensive screening and risk assessment tools, and enabling shared client records through a Community Information Exchange (CIE).
