Partnerships and Projects

As an information and referral organization, Community Connection helps individuals, families, and communities identify, understand, and effectively use the programs that are part of the human service delivery system.

Community Connection has relationships with many service providers in the broader human service system. These relationships help advance an integrated service delivery system that ensures broad access to community services, maximizes the use of existing resources, and facilitates the ability of people who need services to find the most appropriate provider easily.

We categorize our work into these impact areas:

  • Resource Database, Data Sharing and Online Access
  • Disaster Preparedness and Response
  • Healthcare, Paramedics and Social Prescribing
  • Client Database, Ocean Integration and Comprehensive Screening and Risk Rating Scale
  • Community Information Exchange
  • Housing and Homelessness
  • Poverty and Income
  • Seniors and Volunteerism

Land Acknowledgement

For over 15,000 years, Indigenous people walked upon and cared for the lands we now inhabit. The traditional inhabitants of this land, on which we work and live, include the Métis Nation, Anishinaabe, Haudenosaunee, and Wendat Confederacy.  These people and many others have inhabited this land, which was used as hunting and gathering places to support families and communities.

We are grateful for the opportunity to meet, work, and live here, and we thank all the generations of people who have taken care of this land for thousands of years. We acknowledge the impact of colonization and the loss of these lands on Indigenous people.

This recognition is clearly and overtly connected to our collective commitment to make the promise and the challenge of Truth and Reconciliation real in our communities.

Diversity, Equity, and Inclusion

Community Connection is committed to fostering a diverse, equitable, and inclusive organization. We recognize the history of oppression that has resulted in ongoing disparities for the people and communities we serve.

Community Connection’s work is to guide people through the human service system and overcome barriers preventing them from acquiring needed services. We will identify and implement culturally relevant approaches to serving diverse populations and their unique needs.

We believe that by prioritizing engagement with diverse perspectives and evolving our organization to be inclusive and behave equitably, we will improve our services to people and build opportunities for learning, change, and accountability.
