About Us
Community Connection is an internationally accredited community navigation organization dedicated to improving access to services for people across Ontario.
Since our founding in 1969, information and referral service models like ours have evolved significantly, from neighbourhood-based centres to centralized call centres and now to integrated hybrid models.
Today, our approach combines multiple access points, including digital tools such as online directories and self-service resources; call-based support through helplines and live assistance; community engagement through outreach and partnerships; and, where appropriate, in-person service delivery.
This evolution reflects our commitment to meeting people where they are and ensuring flexible, accessible pathways to support.
Core Areas of Work
Community Connection’s work is grounded in three core areas: navigation, curation, and data and insights. Our navigation services connect individuals to the right supports through skilled Community Navigators who provide person-centered information, referral, and system guidance. Our curation work ensures the quality, accuracy, and accessibility of service information through the development and maintenance of comprehensive resource databases. Complementing this, our data and insights (BI) work leverages service interactions and system data to identify trends, inform planning, support partners, and strengthen community responses through evidence-based decision-making.
We are more than a contact centre, we are one of four uniquely positioned nonprofits in Ontario’s community navigation sector—supporting individuals through life’s most challenging moments with empathy, expertise, and unwavering commitment. Our forward evolution is focused on:
Responding to Urgent Needs: Providing immediate support through prevention and diversion services that connect people to help when and where it’s needed most
Supporting Long-Term Change: Offering follow-up services that guide individuals through stages of change, utilizing case coordination skills to address complex, interconnected needs and promote well-being, quality of life, and social inclusion
Removing Systemic Barriers: Partnering collaboratively and drawing on a deep understanding of social barriers, we simplify system navigation and work to eliminate obstacles that prevent access to essential services
Using Data to Drive Solutions: Collecting, analyzing, and reporting data on the demand and supply of human services to identify trends, service gaps, and access barriers—creating opportunities for community partners to co-develop effective, targeted solutions
Maintaining a Trusted Data Foundation: Curating and managing a standardized provincial database of human services that ensures accurate, up-to-date, and consistent information across communities
