´╗┐Community Transportation Link - Simcoe County

  • The Community Transportation Link (CT Link) is a network of community-based transportation service providers developing a web-based Portal that they will use to identify opportunities to pool transportation resources between agencies.
  • The CT Link is funded by the Ministry of Transportation, through the Community Transportation Pilot Grant Program.  Project support is provided by the County of Simcoe and the North Simcoe Muskoka Local health Integration Network.
  • 211 is promoted for people not registered with a transportation agency to learn more about transportation resources available in the community.  http://www.simcoe.ca/dpt/scs/transportationpartnerships

Health Care Referrals – North Simcoe Muskoka LHIN

  • Social determinants of health play a role in increasing risk for the development of chronic diseases.
  • Currently piloting in South Georgian Bay, 211’s Community Navigators receive referrals through the electronic medical record from physicians for patients in need of community support services.
  • 211’s Community Navigator’s provide further assessment of the patient’s needs, ensure they understand the pending referrals, and then conduct a follow up call with the patient to determine if services are in place.

´╗┐´╗┐Homelessness Prevention Fund Initiative – Bruce and Grey Counties

  • A three-month project to provide the intake process for clients applying for United Way's Utility Assistance and Homelessness Prevention Fund for assistance with gas, wood and oil heating, rent and furnace repairs, and conduct follow up surveys to measure outcomes and customized impact measures.

 Refugee and Settlement Directories – Bruce/Grey and Simcoe Counties

  • The United Way of Bruce Grey identified the need to coordinate information about the multiple groups and committees sponsoring and supporting newcomers moving to Bruce and Grey Counties.  Community Connection replicated the project for Simcoe County.
  • Community Connection’s Resource Specialists worked with sponsoring groups to design and develop an online directory which includes contact details about participating groups, as well as programs and services specifically for refugees and newcomers.
  • The next phase of the project will develop an online space for groups to advertise needed items to furnish homes or secure other needed items for newcomers.
